Membership Application

Please read the following information and then click on the appropriate Member Type Link below to be taken to the correlating membership application.

Thank you for your interest in becoming a member of ACPA. Following are the dues amounts and descriptions of the various membership types. Please click on the membership type link that best applies to you in order to apply online. Once your application is complete, including sponsorship and student letter where applicable, the review process takes approximately two weeks.

A sponsor is required for Active, Associate, and Student membership applicants. A sponsor must be a current member of ACPA. You will have an opportunity to submit the name of your sponsor when filling out the online membership application. We will contact the sponsor for verification of support of your application for membership. If you need assistance finding a sponsor, please enter “need sponsor” in the appropriate field when filling out the application. We will either contact you with a list of potential sponsors, or contact one on your behalf.

Individuals applying for Student or International Student membership must provide a letter from their department chair verifying their full time student, resident, fellow, or post-doctoral status. Student letters may be sent by email, fax or mail (see contact information below).

Active Member: Active members have completed a formal training program in their field in the U.S. or Canada, or practice in the U.S. or Canada, and display a professional interest in the study or treatment of cleft lip, cleft palate, and craniofacial anomalies.

Associate Member: Associate membership may be granted to an applicant whose interests are consistent with the goals of the Association, who has displayed an interest in the study or treatment of cleft lip, cleft palate and craniofacial anomalies, who works in the field of cleft lip, cleft palate, and craniofacial anomalies, who resides in the U.S. or Canada, but who does not qualify for Active Member status.

International Member: International membership may be granted to an applicant whose professional interests are consistent with the goals of the Association, and who would otherwise qualify for Active Member status except his/her formal training was completed outside of the U.S. or Canada and he/she practices outside of the U.S. or Canada.

International Associate Member: International Associate membership may be granted to an applicant who would qualify as an Associate Member and who resides outside of the U.S. or Canada.

International Student Member: International Student membership may be granted upon initial application to an applicant whose professional interests are consistent with the goals of the Association, who has displayed an interest in the study or treatment of cleft lip, cleft palate, and craniofacial anomalies, and who is a full-time student in good standing in an undergraduate, graduate, or post-graduate training program outside of the U.S. or Canada. Applicants must provide a letter from their department chair verifying their full time student, resident, fellow, or post-doctoral status.

Student Member: Student membership may be granted upon initial application to an applicant whose professional interests are consistent with the goals of the Association, who has displayed an interest in the study or treatment of cleft lip, cleft palate, and craniofacial anomalies, and who is a full-time student in good standing in an undergraduate, graduate, or post-graduate training program. Applicants must provide a letter from their department chair verifying their full time student, resident, fellow, or post-doctoral status.
