Call for Nominations

2007 Honors of the Association and
2007 Distinguished Service Award
Deadline: October 1, 2006

The ACPA Honors and Awards Committee is soliciting nominations for these two awards.  

The Honors of the Association recognizes an individual whose lifetime of research, healthcare delivery, or leadership has significantly impacted the care of individuals with cleft and craniofacial conditions.

To nominate someone please return only the form below, along with a detailed letter of support to the ACPA National Office.

The Distinguished Service Award recognizes an individual who has demonstrated exceptional service to ACPA.  Nomination Forms and supporting letter instructions can be downloaded below or requested from the ACPA National Office.  Completed materials should be sent to the National Office.

Nomination materials on candidates nominated for either award will be kept on file for a period of two (2) years. Should a candidate not be selected, his or her paperwork will need to be resubmitted with up-to-date information both on the nominating form and in the supporting letter at the expiration of the two year period.
