Frequently Asked Questions

1. What are the fees associated with the approval process?
   
   1. There is a non-refundable initial application fee of $150.00. If your team is approved, this fee will count as your team's first year listing fee.
   2. There is an annual listing fee of $150.00
      
   
   
2. When will we receive feedback from the Commission?
   1. You will be contacted for additional materials needed in support of your application by August 1, 2010.
   2. Teams will be notified of their approval status by November 1 of the year in which they apply.
   
   
3. Who qualifies to sign the application as an "Authorizing Individual?"
   • The person with overall responsibility of the administrative unit under which the team operates. some examples are:
     1. President or CEO of the hospital.
     2. Dean of the university school or department.
     3. President or Board Chair of a corporation.
