Genetics

Below are links to an introductory letter and a genetic screening tool designed for use by Cleft lip/palate teams and Craniofacial Centers by Katherine Medlin of Genzyme Genetics.   The purpose of the screening tool is to provide teams without genetic professionals a mechanism for determining which patients require referral to genetics. This screening questionnaire can be filled out by the patient or can be administered by a health professional.
